<abstract>
Background and Objective: Wilms tumor (nephroblastoma) is the most common renal malignancy in children. Treatment typically involves nephrectomy and chemotherapy, with radiotherapy reserved for selected cases. These treatments may adversely affect renal function. This study aimed to evaluate changes in renal function in children diagnosed with Wilms tumor.
Methods: This retrospective cohort study included patients <18 years who had completed treatment for Wilms tumor at least five years before the assessment. Demographic and clinical characteristics, serum creatinine and urea levels, estimated glomerular filtration rate (eGFR), and renal ultrasonography findings were assessed at baseline and at the last follow-up.
Findings: A total of 32 patients were evaluated. Their mean age was 9 ± 2.6 years, and 20 (62.5%) were female. At diagnosis, 12 patients (37.5%) had stage II disease and 10 (31.3%) had stage III disease. Total nephrectomy was performed in 93.8% of patients, and 7 (21.9%) received neoadjuvant chemotherapy. Four patients (12.5%) died during follow-up; these deaths were not directly attributable to renal dysfunction. At baseline, 4 patients (12.5%), 26 (81.2%), and 2 (6.3%) were classified as having stage 1, stage 2, and stage 3 chronic kidney disease (CKD), respectively, based on eGFR. At the five-year assessment, the corresponding numbers were 2 (6.3%), 22 (68.7%), and 8 (25.0%), respectively. Contralateral kidney hypertrophy was observed in 25% of patients during follow-up.
Conclusion: Renal function may deteriorate over time in children treated for Wilms tumor, underscoring the importance of long-term follow-up and regular monitoring of renal function.
</abstract>
