Background and Objective: The size of a newborn at the time of birth is decided by the rate of growth of the fetus which is dependent on several factors. The simplest technique for defining small-for-gestational age (SGA) at birth is a growth chart. The present study was aimed to compare Fenton and Intergrowth-21 charts to assess the size at birth in preterm newborn to estimate the prevalence of small for gestational age infants among preterm.
Methods: The study was conducted on preterm neonates (32 to 36 weeks 6 days) in Chennai whose anthropometric measurements (head circumference, weight and length) were recorded within 24 hours of birth. The measurements were plotted in both Fenton and Intergrowth-21 Charts. The size of new born was interpreted and compared between both charts.
Findings: As per the Fenton chart microcephaly was seen in 12 %, short stature in 24.8%, small for gestational age in 19.8% while higher rates were found by Intergrowth 21 chart for microcephaly, macrocephaly, tall stature and large for gestational age. There was good agreement between Fenton chart and Intergrowth 21 charts for head circumference (.kappa 0.7 and 0.8) and substantial agreement for length and weight (kappa 0.5 and 0.7).
Conclusion: In our study population, the incidence of small for gestational age in preterm newborn was 19.8% to 13.6%, as assessed by Fenton and Intergrowth-21 curves with substantial agreement between these curves. Hence our study concludes that both Fenton and Intergrowth-21 charts can be used to identify small for gestational age at birth in preterm newborn.
